If any one of these elements is weak, the process risks becoming nothing more than a collection of administrative tasks: updating an SOP, retraining staff, sending a reminder to the supplier, and closing the case. <\/p>\n\n<p class=\"link-verde has-medium-font-size wp-block-paragraph\">The goal, however, is to understand why the event occurred and to take action on the system that was supposed to prevent or detect it. The<a href=\"https:\/\/www.fedlex.admin.ch\/eli\/cc\/2018\/786\/en\"><strong>Ordinance on the Authorization of Medicines<\/strong><\/a> stipulates that manufacturing must comply with the GMP rules set forth in the relevant annexes. For wholesale distribution, Swissmedic refers to the applicable GDP requirements through Article 15 and Annex 4 of the ordinance.  <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">During GMP and GDP inspections, Swissmedic may examine, among other aspects, deviation management, documentation, training, monitoring of storage conditions, supplier qualification, complaints, outsourced activities, and self-inspections. Re-inspections may also verify the corrective actions required during a previous visit. <\/p>\n\n<p class=\"link-verde has-medium-font-size wp-block-paragraph\">The <a href=\"https:\/\/picscheme.org\/docview\/6606\"><strong>PIC\/S GMP Guide PE 009-17<\/strong><\/a> requires an appropriate level of root cause analysis, corrective or preventive actions consistent with the findings of the investigation, and monitoring of their effectiveness. It also specifies that attributing the event to human error must be justified, after first verifying that no process, procedural, or system issues have been overlooked. <\/p>\n\n<p class=\"link-verde has-medium-font-size wp-block-paragraph\">Tools such as the 5 Whys, the Ishikawa diagram, and the SMART framework are <strong>best practices<\/strong>, not specific legal requirements. Also, <a href=\"https:\/\/www.iso.org\/standard\/62085.html\"><strong>ISO 9001:2015<\/strong><\/a> is a voluntary standard, unless it becomes applicable due to a company\u2019s choice, certification, contract, or other provision. Revising the process for distributing, collecting, and checking copies, on the other hand, is a corrective action. <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Under ISO 9001:2015, prevention is integrated into risk-based thinking and is no longer a separate clause. In the pharmaceutical sector, however, the term CAPA typically continues to encompass both corrective and preventive actions. <\/p>\n\n<div style=\"height:35px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<h2 class=\"wp-block-heading has-large-font-size\">How to Properly Describe a Nonconformity<\/h2>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">An effective description must include verifiable facts, not hypotheses about the cause. According to the ISO\/IAF guidance, a well-documented nonconformity includes the unmet requirement, objective evidence, and a clear statement of the deviation. <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">A practical structure is:<\/p>\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"has-medium-font-size wp-block-paragraph\">On <strong>[date and time]<\/strong>, during the process or at the location <strong>[where]<\/strong>, <strong>[measurable fact]<\/strong> was observed regarding <strong>[products, lots, documents, or records involved]<\/strong>, in violation of <strong>[requirement, SOP, specification, or authorized condition]<\/strong>, as demonstrated by <strong>[evidence]<\/strong>.<\/p>\n<\/blockquote>\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Example \u2013 Temperature Deviation<\/strong><\/p>\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"has-medium-font-size wp-block-paragraph\">On July 14, 2026, the L-03 data logger in refrigerator F-02 recorded a maximum temperature of 11.8 \u00b0C for 47 minutes, compared to the expected range of 2\u20138 \u00b0C. Batches A and B were present in the refrigerator. The products were placed in quarantine pending a documented impact assessment. <\/p>\n<\/blockquote>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The description should not jump to the conclusion that \u201cthe operator left the door open\u201d until the hypothesis has been verified.<\/p>\n\n<div style=\"height:35px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<h2 class=\"wp-block-heading has-large-font-size\">Assess Criticality, Impact, and Urgency<\/h2>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The initial assessment is used to determine containment measures, the scope of the investigation, responsibilities, and the timeline for handling the situation. It may consider: <\/p>\n\n<ul class=\"wp-block-list has-medium-font-size\">\n<li>potential consequences for patients, users, and product quality;<\/li>\n\n\n\n<li>impact on identity, effectiveness, security, traceability, or data integrity;<\/li>\n\n\n\n<li>the number of products, processes, locations, or records involved;<\/li>\n\n\n\n<li>the likelihood that the problem has already occurred elsewhere;<\/li>\n\n\n\n<li>the ability to detect the error before use or dispensing;<\/li>\n\n\n\n<li>frequency and presence of preceding signals.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Urgency does not always equate to criticality. An easily traceable data entry error may require immediate correction, even if its impact is limited. Conversely, an outdated SOP may not yet have caused any harm, but it could indicate a systemic weakness affecting multiple departments.  <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">There is no one-size-fits-all framework that applies to all organizations. Criteria, categories, and thresholds must be defined internally, applied consistently, and commensurate with the risk. <\/p>\n\n<div style=\"height:35px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<h2 class=\"wp-block-heading has-large-font-size\">How to Conduct a Root Cause Analysis<\/h2>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Root cause analysis does not begin by selecting a tool, but by reconstructing the event. It is necessary to gather a timeline, recordings, technical data, documented accounts, training information, interviews, previous deviations, and operating conditions. <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">It is helpful to distinguish between:<\/p>\n\n<ul class=\"wp-block-list has-medium-font-size\">\n<li><strong>immediate cause:<\/strong> what directly brought about the event;<\/li>\n\n\n\n<li><strong>root cause:<\/strong> a weakness that allowed the immediate cause to manifest itself;<\/li>\n\n\n\n<li><strong>Contributing factors:<\/strong> conditions that increased the likelihood or impact.<\/li>\n<\/ul>\n\n<div style=\"height:35px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<h3 class=\"wp-block-heading has-medium-font-size\">When to Use the 5 Whys<\/h3>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The 5 Whys method is suitable for relatively straightforward and well-defined problems.<\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Example \u2013 Obsolete SOP<\/strong><\/p>\n\n<ol class=\"wp-block-list has-medium-font-size\">\n<li>Why was an outdated SOP used?<br\/>Because a hard copy was still there in the department.<\/li>\n\n\n\n<li>Why hadn&#8217;t it been withdrawn?<br\/>Because the department was not included in the list of distribution points.<\/li>\n\n\n\n<li>Why was the list incomplete?<br\/>Because the copies were printed locally without being registered.<\/li>\n\n\n\n<li>Why was it possible to print unchecked copies?<br\/>Because the document management system did not apply watermarks or restrictions.<\/li>\n\n\n\n<li>Why hadn&#8217;t the risk been identified?<br\/>Because the document review process verified approval but not actual use at the operational sites.<\/li>\n<\/ol>\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The cause is not simply \u201cfailure to return the copy,\u201d but the lack of effective oversight of the copies in use.<\/p>\n\n<div style=\"height:35px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<h3 class=\"wp-block-heading has-medium-font-size\">When to Use the Ishikawa Diagram<\/h3>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The Ishikawa diagram is useful when an event can stem from multiple categories of causes: people, methods, equipment, materials, measurements, and the environment.<\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">For a temperature deviation, the following could be analyzed simultaneously:<\/p>\n\n<ul class=\"wp-block-list has-medium-font-size\">\n<li>duration and frequency of door opening;<\/li>\n\n\n\n<li>probe positioning and calibration;<\/li>\n\n\n\n<li>refrigerator contents;<\/li>\n\n\n\n<li>alarm configuration;<\/li>\n\n\n\n<li>absence of the primary person in charge;<\/li>\n\n\n\n<li>organization of picking activities;<\/li>\n\n\n\n<li>ambient temperature.<\/li>\n<\/ul>\n\n<div style=\"height:35px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<h3 class=\"wp-block-heading has-medium-font-size\">When to analyze contributing factors<\/h3>\n\n<p class=\"link-verde has-medium-font-size wp-block-paragraph\">In complex events, there isn&#8217;t always a single cause. The <a href=\"https:\/\/committee.iso.org\/files\/live\/sites\/tc176\/files\/PDF%20APG%20New%20Disclaimer%2012-2023\/ISO-TC%20176-TF_APG-ReviewNonconformity.pdf\"><strong>ISO\/IAF guidance on reviewing and closing nonconformities<\/strong><\/a> recommends not stopping at the first factor identified and distinguishing between possible hypotheses and those that have been confirmed. <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">This approach is particularly useful for:<\/p>\n\n<ul class=\"wp-block-list has-medium-font-size\">\n<li>recurring complaints with different causes;<\/li>\n\n\n\n<li>dispensing or recording errors;<\/li>\n\n\n\n<li>training issues;<\/li>\n\n\n\n<li>defects originating from suppliers;<\/li>\n\n\n\n<li>Deviations influenced by workload, IT interfaces, and organizational factors.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">\u201cOperator inattention\u201d or \u201cinsufficient training\u201d are rarely sufficient conclusions. We must ask ourselves why the system allowed the error to occur, why it was not detected, and whether the task was designed in a clear and sustainable manner. <\/p>\n\n<div style=\"height:35px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<h2 class=\"wp-block-heading has-large-font-size\">Define SMART actions, responsibilities, and deadlines<\/h2>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">An action must be:<\/p>\n\n<ul class=\"wp-block-list has-medium-font-size\">\n<li><strong>specific:<\/strong> describes exactly what will change;<\/li>\n\n\n\n<li><strong>measurable:<\/strong> defines expected results and evidence;<\/li>\n\n\n\n<li><strong>feasible:<\/strong> has the necessary resources and expertise;<\/li>\n\n\n\n<li><strong>relevant:<\/strong> it addresses the confirmed cause;<\/li>\n\n\n\n<li><strong>Time-limited:<\/strong> It has a justified expiration date.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">\u201cRestructuring the workforce\u201d is too vague. A better way to put it is: <\/p>\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"has-medium-font-size wp-block-paragraph\">By September 30, the operations manager will update the recording instructions, implement an automated check for required fields, and verify through practical observation that all relevant staff members know how to complete and correct the records in accordance with the new procedure.<\/p>\n<\/blockquote>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Each activity must have a designated or functional manager with the necessary authority, a deadline, a documented output, and clearly defined dependencies. A SOP may have been approved and training completed without any real change in operational behavior. <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The verification plan should be defined prior to implementation and should specify:<\/p>\n\n<ul class=\"wp-block-list has-medium-font-size\">\n<li>indicator to be checked;<\/li>\n\n\n\n<li>initial situation or baseline;<\/li>\n\n\n\n<li>expected result;<\/li>\n\n\n\n<li>time period and sample size;<\/li>\n\n\n\n<li>audit manager;<\/li>\n\n\n\n<li>evidence to be preserved;<\/li>\n\n\n\n<li>Decision to be taken in the event of a negative result.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The period should not be automatically set to 30, 60, or 90 days. It must be long enough to observe the process over a representative number of cycles. <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Examples of assessments:<\/strong><\/p>\n\n<ul class=\"wp-block-list has-medium-font-size\">\n<li><strong>Temperature deviations:<\/strong> analysis of subsequent shipments or opening cycles, alarm response times, and the absence of events attributable to the same cause.<\/li>\n\n\n\n<li><strong>Out-of-date SOPs:<\/strong> Checking points of use, ensuring there are no obsolete copies, and verifying compliance during a subsequent self-inspection.<\/li>\n\n\n\n<li><strong>Training:<\/strong> practical observation or competency test\u2014not just signing the logbook.<\/li>\n\n\n\n<li><strong>Suppliers:<\/strong> compliance of subsequent deliveries, adherence to change notices, and verification of agreed-upon actions.<\/li>\n\n\n\n<li><strong>Recurring complaints:<\/strong> trends normalized relative to distribution volumes, broken down by product, lot, cause, and channel.<\/li>\n\n\n\n<li><strong>Exemption or registration:<\/strong> targeted audit, process observation, and verification of the proper implementation of the barriers put in place.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The mere absence of new deviations may not be very meaningful when the event is rare. It is preferable to combine this with process indicators, such as proper use of the procedure, timeliness of alarms, or completeness of records. <\/p>\n\n<div style=\"height:35px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<h2 class=\"wp-block-heading has-large-font-size\">Complete CAPA Example: Temperature Deviation<\/h2>\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Non-compliance:<\/strong> The F-02 refrigerator exceeded 8 \u00b0C for 47 minutes. Two batches were present in the unit. <\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Correction and containment:<\/strong> products placed in quarantine; verification of the data logger; collection of stability data; temporary suspension of refrigerator use.<\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Impact Assessment:<\/strong> Quality Assurance evaluates duration, maximum temperature, product characteristics, available data, and the potential involvement of other units.<\/p>\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Root Cause Analysis:<\/strong> The alarm had been triggered but was sent only to the phone of the manager, who was absent. The prolonged opening of the door during the picking process caused the temperature to rise.
